Birth Certificates
Terrebonne General’s Medical Records Department assists parents with applying for their newborn’s birth certificate.
After your baby’s birth, you will receive a call from our Birth Certificate Clerk who will request information from you according to the State of Louisiana Vital Records Department’s rules and regulations. This information will be used to create your child’s birth certificate. Upon completion of the signed birth certificate, a complementary birth verification letter to use as verification of birth will be given to you until you receive your certified copy of the baby’s birth certificate from the Louisiana Office of Vital Records (usually 3-6 months after birth).
